The Captain of the Guard is supposed to drop a Stone Key which is what you need to get into the Reaver Castle.
Spoke with Jutebox in the game, and he said it was supposed to drop every time.

I checked MQEmulator, even though it is the PEQ database, and it only has a 35% chance to drop.
I tend to think it should drop every time. But I have killed Captain of the Guard like 20 times, and haven't seen one yet.
